What is an Impacted Tooth?

You may have heard this term the first time you or a friend got your wisdom teeth. That makes sense, as wisdom teeth are the teeth most often impacted in teenagers and young adults. But other permanent teeth can be impacted as well. What exactly do we mean by “impacted teeth,” and what can we do to treat them?

The term “impacted” means that somehow a tooth has been blocked from erupting properly. A tooth may be completely blocked by another tooth, erupt in the wrong space, or even come in from the wrong direction. Depending on the teeth involved, there are several different options for treatment.

From Baby Teeth to Permanent Teeth

Normally, when children lose a baby tooth, a permanent tooth is right there, ready to take its place. But teeth don’t always behave according to plan. Occasionally, that baby tooth just won’t budge, and the permanent tooth starts to erupt behind it. When this happens, a simple baby tooth extraction will often let the permanent tooth move into its proper position on schedule.

A more complicated situation develops when upper teeth are impacted because there isn’t enough space in the mouth for them. In this case, a device called a palatal expander might be used to gradually widen the upper jaw to allow the permanent teeth to erupt without crowding.

In other rare cases, a tooth (often the canine) fails to erupt and may require oral surgery to uncover it, followed by orthodontic treatment to guide it into position.

Impacted teeth can result from other causes as well, and every impacted tooth should be treated as quickly as possible. Left untreated, the teeth can fail to erupt at all or erupt in the wrong place, crowd other permanent teeth, damage the roots of the teeth near them, and lead to difficulties eating and dental pain.

Wisdom teeth

Wisdom teeth are often a problem because there is simply not enough room in the jaw for them.

Wisdom teeth that are completely impacted (still in the jawbone) can sometimes be left alone if they aren’t causing other problems. But if impacted wisdom teeth develop cysts, affect the teeth around them, or lead to other dental complications, they should be extracted.

Partially erupted teeth, those that have begun to emerge through the gums but don’t erupt fully, can be the source of different gum and tooth problems. Because the gum tissue overlaps the tooth, food particles and bacteria can become trapped, leading to rapid tooth decay and even infection. In this case, extraction is probably the best option.

Be Proactive

The term “impacted” actually comes from the Latin root meaning “pushed against.” But teeth that don’t erupt at the right time, in the right place, can have a different kind of impact on dental health and appearance. And the earlier we can catch these problems, the easier it is to treat them.

Regular exams and X-rays with Dr. Timm at our Michigan Center, MI office will show the progress of the teeth even before they erupt, and if there will be the space for them to fit in the mouth properly. We may recommend a visit to the orthodontist by the age of seven to see if there are any signs of potential orthodontic problems.

Intervention at an early stage can prevent potential problems from becoming major ones. That is why it’s so important to be proactive when teeth are erupting in children and young adults. After all, a healthy, confident smile makes a real impact!

Carbs and Cavities

The Good News

Carbohydrates are one of the body’s essential macronutrients (along with protein and fat). We use carbs to convert the food we eat into energy. How does this work? It’s a sophisticated process:

  • Carbs break down into sugars as we digest them
  • Sugars are absorbed into our bloodstream
  • The pancreas releases insulin when blood sugar levels rise
  • Insulin enables sugars to move from our blood to our cells
  • Cells throughout the body use this sugar for energy.

Without the necessary amount of carbohydrates, our bodies lose a vital source of energy. So, why are carbs a dental concern?

The Bad News

Some foods immediately begin breaking down into sugars in the mouth. Sugars are a favorite food source for the oral bacteria that form plaque. They use this sugar to produce the acids that weaken our enamel and lead to cavities. And the more often we eat these foods, and the longer they remain in the mouth, the more damage our enamel suffers.

But there’s a silver lining! We can be healthier physically and get a jump on preventing damage from sugary treats by becoming more discriminating in our choice of carbs and timing our indulgences wisely.

Good Carb/Bad Carb

Unprocessed, complex carbohydrates are found in foods like whole-grain breads and cereals, legumes, and vegetables. They contain the vitamins, minerals, and fibers which are lost when foods are refined. They are composed of larger, more complex molecules, and so they break down gradually for sustained energy.

Some simple carbohydrates break down into sugars more quickly, but also offer important vitamins, minerals, and fiber. Fruits and dairy products, for example, are an important part of a balanced diet.

But some carbs are not pulling their nutritional weight. Refined sugars (think candies, desserts, and sodas) and refined starches (white bread and rice, potato chips, pastries) break down quickly into sugar in the mouth. Worse, many of these foods tend to stick around. Sticky sugars and sticky starches cling to our enamel and hide between the teeth and in the crevices of our molars. Not only do these treats provide a sugary feast for acid-producing bacteria, they take their time doing it!

So, What to Do?

If you have a diet filled with healthy carbs (whole grains, fruit and vegetables, legumes, dairy products), you’re already on the right track. Kudos! But does this mean no desserts? Ever?

No! We all need a cookie sometimes. But you can decrease the chance of enamel damage by interrupting the carbs to cavities cycle.

First, if you are indulging in a rich dessert or some salty chips, better to do it as part of a meal. When you eat a full meal, your body produces more saliva. Saliva not only helps wash away food particles, it also helps neutralize the acids that damage enamel.

Secondly, if you eat simple carbs and sugars all day, your mouth and teeth are being treated to acids all day. If you are going for a snack, there are many great options that don’t use refined sugars and starches. Think fruit smoothies (with a big dollop of vegetables) or whole-grain crackers with hard cheese instead of a can of soda and a bag of pretzels.

Good Teeth Lead to Sporting Success

You already know that taking care of your teeth can help prevent tooth decay and the need for extensive work such as root canals or implants, which can be inconvenient and expensive. But the benefits of good teeth can go far beyond having an attractive smile and being able to crunch carrots and chew meat.

The American Dental Association explains that healthy teeth are linked to a lower risk for heart disease and diabetes. Furthermore, recent research published in the British Journal of Sports Medicine states that good teeth can improve athletic performance among elite athletes.

Researchers examined the oral health of nearly 300 athletes in 25 sports at the 2012 Olympics in London. They looked for conditions such as dental caries, gingivitis, dental erosion, and periodontal disease, and asked about recent visits to a dentist.

Study investigators also asked athletes whether their oral health interfered with quality of life or athletic training and performance. The study concluded that poor oral health and fewer dental visits led to interference with preparation for competition.

This can happen for a few reasons. Tooth pain can disrupt sleep, which leads to slower reaction times. Oral health conditions can indicate chronic inflammation in the body, which means suboptimal performances on an elite level. Tooth pain can interfere with focus during training and competition.

Unfortunately, merely taking good care of your teeth won’t turn you into an Olympic gold medalist. However, the benefits can still be worthwhile. Even if healthy teeth provide little if any detectable gain in your athletic abilities, the potential benefits of maintaining a healthy mouth clearly go far beyond an attractive smile.

Practicing good oral hygiene and seeing Dr. Timm regularly can promote your physical health, and maybe – just maybe – you will start to achieve an advantage over your weekend athletic opponents.

Outside activities? You slather your kids with sunscreen. Biking? You don’t let your child leave the house without a helmet. Youth sports? You provide mouthguards and padding and headgear and all the other tools designed to keep your child safe. Protecting your child is a fundamental part of parenthood, and you take your job seriously.

Protecting your child’s dental health is fundamental, too! Tooth decay is the most common chronic disease in children, and children’s premolars and molars are far more vulnerable to decay than any of their other teeth. You can help protect your child’s molars and premolars with a simple and effective treatment—dental sealants.

  • How Do Sealants Work?

The bacteria in plaque use food particles to create acids. These acids erode the minerals in tooth enamel, creating weak spots that become cavities over time. Molars and premolars are especially vulnerable to cavities because of their uneven chewing surfaces. The dips and grooves on top of the teeth—technically known as “pits and fissures”—collect food particles and bacteria, and can be difficult for children to clean completely when brushing. That’s why cavities are so common in newly erupted molars.

Dental sealants protect your child’s molars and premolars by creating a barrier that covers and smooths out the chewing surface of the tooth, preventing bacteria and food particles from getting stuck inside those uneven pits and fissures.

  • What Kind of Sealants Are Available?

The two most common dental sealants are composite resin coatings and glass ionomer sealants.

With resin sealants, after each tooth is cleaned and dried, an etching solution will be brushed onto the top surface of the molar. This etching slightly roughens the surface so that the sealant will stick to the tooth more effectively. A thin coat of the sealant is then painted on and hardened under a special curing light.

Glass ionomer sealants use a flexible paste that bonds to the tooth and hardens within minutes. While they generally don’t last as long as resin sealants, they are designed to absorb and release fluoride for extra cavity-fighting protection.

  • Do Sealants Work?

They certainly do! According to the Centers for Disease Control, sealants can prevent 80% of the cavities in molars and premolars, which is where 90% of children’s cavities appear. Dental sealants can last from three to five years, or even longer. Dr. Timm will check the condition of any sealants at each dental exam, and let you know if it’s time for a reapplication.

  • When’s the Best Time to Get Sealants?

Tooth enamel gets harder as we get older, so children’s newly erupted teeth are more at risk for cavities. First adult molars usually arrive when a child is six to seven years old, and second adult molars come in around the age of 12. The first and second premolars can erupt between the ages of ten to 12. As soon as the first permanent molars begin to erupt, it’s a good time to talk to Dr. Timm about the best time to apply sealants.

And what about baby teeth? Even though baby teeth are meant to be replaced, they shouldn’t be lost to tooth decay before they are ready to fall out. Primary teeth help young children learn to speak and eat properly and hold the place for adult teeth so these permanent teeth come in where they should. Baby teeth have thinner enamel, and so cavities can progress more quickly. Dr. Timm can let you know if sealant protection for your child’s baby molars is indicated.
